March 23, 2020, President Donald Trump issued Executive Order 13911 and invoked the Defense Production Act ban from exporting critical medical personal protective equipment (PPE) used to protect wearers from the coronavirus known as SARS-CoV-2, including surgical masks, masks that provide a physical barrier to fluids and particulate materials, N-95, N99, N100, R95, R99, R100, or P95, P99, P100 filtering respirators and medical gloves.  It is the policy of the United States to prevent domestic brokers, distributors, and other intermediaries from acquiring these items for the purpose of resale at prices in excess of prevailing market prices. It is the policy of the United States to prevent domestic brokers, distributors, and other intermediaries from diverting such material overseas. 
I certify that my order for personal protective equipment does not violate US Executive order 13911 as described above.

IMPORTANT: infection control and PPE terms and conditions during the Global Pandemic.

All infection control products including any germicidal cleaners, alcohol wipes, disinfecting wipes, hand sanitizers, personal protection items including masks and gloves are non-cancelable, non-returnable, non-refundable and must be paid for when the order is placed. This is the policy of all our infection control manufactures so it is also our policy during Coronavirus disease (COVID-19) preparation and control effort.  The customer agrees not to request a refund nor attempt to cancel, modify, or return a pre-order.  

State Surgical Supply operates on extremely low profit margins to be an online low-price leader and have not increase profit margins on any items. During the national emergency, preordered items are subject to retroactive manufacturer price increases.  Due to the enormous increase in production and import cost, manufacturers have been forced to increase prices retroactively on items that have not shipped. If your preordered items are subject to a retroactive price increase you will have the option to cancel the item for a full refund or you can keep the order in place at the new price. 

Preordered items are subject to being discontinued which would result in a canceled order and a refund in full.  State Surgical Supply is not responsible for product shortages, discontinuation of products.

 

There is a high return rate on parcel shipments due to businesses that are shut down due to COVID-19.   Customers will be responsible for the return shipment and processing costs due to not being available to receive orders. This cost can be significant.  We strongly encourage you not to order if there is any chance you will not be there to accept delivery.

Recent developments regarding the 2020 & 2019-nCoV virus (coronavirus) have created large surges in market demand for many Personal Protection Equipment (PPE) like mask and gowns and infection control products. Mask manufacturers are not providing delivery estimates so we cannot provide estimates either.

National Emergency Events

National emergency events can cause shortages on medical supplies. These items are often allocated to allow more customers to receive product and to prevent hoarding of medical supplies in short supply.

Signature is not required on orders less than $750

To keep shipping cost low for our customers, we follow the PayPal standard regarding signature requirements for UPS, FedEx or USPS package receipt.  If you feel a signature should be required for your package delivery, you must inform us in the comment at check out.  If you do not make the special request, then you agree to accept the standard delivery receipt as proof of delivery.

State, local and import taxes:
We only collect and submit Arkansas, California and Oklahoma sales tax on orders shipped to the state of Arkansas, California or Oklahoma.  The customer is responsible for filing and submitting any import or state/local sales tax to the proper taxing authority for orders shipped outside of the State of Arkansas, California or Oklahoma.

I affirm that my purchase today of needles, syringes and all other products are for legitimate legal use and I verify that I am OVER 18 years of age.

If purchasing any item identified as "healthcare professional only", I affirm that I am a healthcare professional.

Returns and Repairs

Please keep all packing material and documentation if your equipment has to be serviced or returned.

Some non-defective products may be returned for credit or exchange. Returns authorized by State Surgical Supply are subject to a restocking fee of up to 30% and must be completed within 30 days of the date of invoice. The returned products must be in resalable condition and must include all original packaging and accessories.

Before returning any product, you must obtain a Return Merchandise Authorization (RMA) number. NO returns, of any type, will be accepted without an RMA number. To obtain one, please call Customer Service at 800-756-6871 and have the following information on hand:

  1. Customer Name
  2. invoice Number
  3. Serial Number
  4. Any problems with the product

All defective products will be accepted by State Surgical Supply for credit, exchange, or repair at State Surgical Supply's discretion within 30 days from the invoice date. After 30 days, the manufacturer's warranty applies.

RX products, sterile products and certain hygienic products such as raised toilet seats cannot be returned into the supply chain and are not returnable.

Due to manufacturers' return policies some equipment and all custom equipment may be returned to State Surgical Supply for repairs only - no refunds or exchanges can be made at any time

Pricing and Availability

For all prices and products, we reserve the right to adjust due to errors, market conditions, product discontinuation, typographical errors in content, typographical errors in advertisements or retroactive manufacturer price increases.  Thomas Distribution Solutions, LLC AKA States Surgical Supply is not responsible for future or retroactive manufacturer price increases on items already on order, which may occur at any time or without notice.  Products may be discontinued at any time and without notice causing orders to be canceled.

The product images on this web site may not exactly reflect the product you receive. Design revisions and color variations may exist. We keep the most recent images provided by the manufacturer; however, we provide no guarantee that the image is accurate. Please be aware that the manufacturers of some products use generic image for certain products.

IN NO EVENT SHALL THOMAS DISTRIBUTION SOLUTIONS (AKA STATE SURGICAL SUPPLY), ITS SUBSIDIARIES OR AFFILIATES, OR THEIR RESPECTIVE PARTNERS, OFFICERS, DIRECTORS, EMPLOYEES, SHAREHOLDERS, REPRESENTATIVES OR AGENTS (COLLECTIVELY, "STATE SURGICAL SUPPLY") BE LIABLE FOR DIRECT, SPECIAL, INCIDENTAL, CONSEQUENTIAL, PUNITIVE, OR INDIRECT DAMAGES (INCLUDING BUT NOT LIMITED TO, LOSS OF USE OR PROFITS), HOWEVER CAUSED AND WHETHER OR NOT STATE SURGICAL SUPPLY HAS BEEN ADVISED OF THE POSSIBILITY OF ANY SUCH DAMAGES. YOU AGREE THAT STATE SURGICAL SUPPLY'S MAXIMUM LIABILITY ARISING FROM ANY PRODUCT SOLD BY STATE SURGICAL SUPPLY SHALL NOT EXCEED THE PRICE OF SUCH PRODUCT.
